The following terms & conditions apply:
- Scholarships will be limited to cancer survivors and healthcare professionals who play an integral role in the cancer community (e.g., oncology nurses, social workers or patient navigators).
- Travel Scholarships for the Midwest Conference on Work & Cancer are limited to residents of Midwestern states.
- The Cancer and Careers staff is fair and equal in its scholarship application review and award processes for all persons, without regard to race, color, religion, sex, national origin, disability, veteran status, sexual orientation or any other characteristic protected by law.
- All scholarship decisions are final. There is no appeals process for decisions made regarding the awarding of these scholarships.
- Submitting a scholarship application does not guarantee a scholarship will be granted.
- Scholarship recipients agree to provide to Cancer and Careers a testimonial regarding their experience, for use in reports and future promotional materials. A testimonial form and instructions for completion will be emailed them the week after the event. Names will be kept confidential upon request.
- Scholarship recipients agree to participate in follow-up surveys.
- Scholarships may include coverage of travel and/or lodging expenses. Details regarding expenses eligible for reimbursement will be included in the award letter.
- CAC will reimburse eligible expenses up to the awarded amount for those who attend the conference and provide the required receipts, testimonial, etc. Eligible expenses will be indicated in recipients’ individual award letters.
- Reimbursements will be made only if the recipient attends the entire 2020 Midwest Conference on Work & Cancer, in Chicago, on March 27, 2020.
- Please note: Past recipients of a travel scholarship for the National Conference on Work & Cancer are not eligible for scholarships for the Midwest Conference on Work & Cancer.
Travel Scholarships
- Scholarship recipients who receive travel as part of their award are responsible for arranging and paying for their own travel up-front.
- A copy of travel receipts must be sent to CAC upon booking, or no later than 11:59 PM CT on Monday, April 6, 2020. Reimbursement checks will be mailed the week of April 27, 2020. If this reimbursement policy creates a financial hardship for a scholarship recipient, he/she should contact Sissy Silva, Program Coordinator, at 646-929-8040.
- CAC highly recommends scholarship recipients purchase travel insurance.
Lodging Scholarships
- Scholarship recipients who receive lodging as part of their award are responsible for arranging and paying for their own accommodations up-front.
- A copy of hotel receipts must be sent to CAC upon booking, or no later than 11:59 PM CT on Monday, April 6, 2020. Reimbursement checks will be mailed the week of April 27, 2020. If this reimbursement policy creates a financial hardship for a scholarship recipient, he/she should contact Sissy Silva, Program Coordinator, at 646-929-8040.
- CAC will only reimburse for the room and associated room taxes. Any incidentals (e.g. room service, minibar purchases, parking) or gratuities are not eligible for reimbursement.
